Cody Daniel Fugate

Cody Daniel Fugate, 29, of Middletown, died Friday, April 1, 2016, at Miami Valley Hospital. He was born in Jacksonville, Florida on January 25, 1987 to parents Terry Callahan and Kelly Lynn Lewis.

Cody was a devoted father to his children and loved his family. He was an avid sports fan and coached little league. He enjoyed singing, playing guitar and spending time with his friends in Madison Township. Cody worked as a maintenance mechanic for Gray America Corp. in Moraine.

Cody is survived by his parents; son, Landen Fugate; daughter, Kaylee Fugate; brother, Garrett Lewis; sisters: Brittney Lewis, Tiara Callahan, Jennifer Stidham and Keisha Callahan; step brother, Zach Beard; maternal grandparents, Arnold and Rhonda Fugate; maternal grandmother, Rebecca Dennis; paternal grandmother, Shirley Callahan and aunt, Patsy Stugmeyer.

Cody was preceded in death by his paternal grandfather, Brent Callahan; paternal grandmother, Charlotte Henderson and great grandparents, Daniel and Estelle Fugate.

Funeral Service will be held Wednesday, April 6, 2016 at 1:00 pm at WILSON-SCHRAMM-SPAULDING FUNERAL HOME, 3805 Roosevelt Blvd., Middletown with Reverend Dr. Roger Green officiating. Visitation will be Tuesday from 5:00 - 8:00 pm. Interment will be at Pleasant Hill Cemetery, Madison Township.
