Phyllis J. Nichols, 79, of Middletown, died Saturday, December 9, 2017, at Hospice of Butler & Warren Counties. She was born in Middletown on October 31, 1938 to parents Donald and Louella (Hasselback) Forster.
Phyllis had worked as a registered nurse for Middletown Regional Hospital for 30 years, then part-time at Kettering Hospital for 7 years. She was a very active member of Holy Family Parish.
Mrs. Nichols is survived by her husband, Larry Nichols; son, Gary David (Sarah) Adams; daughters, Helen (Mike) Svarda & Elizabeth Frisch; brother, James (Wanda) Forster; five grandchildren, Melissa (Scott) Jones, Britney (Ejay) Edquid, Asher, Aiden & Adlai Adams; five great grandchildren, Noah, Mica, & Madelyn Jones, Ryder & Raelyn Edquid; niece, Shannon Forster; nephew, James Max Forster; and numerous cousins. She was preceded in death by her parents.
Mass of Christian Burial will be held Saturday, December 16, 2017 at 10:00 am at Holy Family Parish - St. John's Church, 1405 First Avenue, Middletown with Father John Civille as celebrant. Visitation will be Friday from 4:00 - 7:00 pm at Wilson-Schramm-Spaulding Funeral Home, 3805 Roosevelt Blvd. at Bonita Dr., Middletown. Interment will be at Springboro Cemetery.
In lieu of flowers the family asks that memorial donations be made to Hospice of Butler & Warren Counties, 5940 Long Meadow Dr., Franklin, OH 45005.
