Phyllis J. Smith

Phyllis J. Smith, 70, of Middletown, died Tuesday, December 13, 2011, at her home. She was born in Middletown on August 8, 1941 to parents Walter and Frieda (Dudley) Hoffman.

Phyllis graduated from Bishop Fenwick High School in 1959 and worked as a substitute secretary for Middletown City Schools and later worked for Clark, Schaefer & Hackett.

Mrs. Smith is survived by her husband, Gene Smith; five sons, Robert (Pam) Lundy, Mike Lundy, Paul (Patti) Lundy, Charlie (Jenny) Smith & Mike G. (Lisa) Smith; sister-in-law, Joan Hoffman; 17 grandchildren; and 20 great grandchildren.

She was preceded in death by her son, Billy Smith; parents; and brother, Joseph Hoffman.

The family wishes to thank Dr. Omer Hurlburt III, Dr. Albert Malcolm and Hospice Care of Middletown for their wonderful care.

Mass of Christian Burial will be held Saturday, December 17, 2011 at 10:00 am at Holy Family Parish - Holy Trinity Church, 201 Clark Street, Middletown with Father John Civille as celebrant. Visitation will be Friday from 5:00 - 8:00 pm at Wilson-Schramm-Spaulding Funeral Home, 3805 Roosevelt Blvd., Middletown. Interment will be at Woodside Cemetery.

In lieu of flowers memorial donations may be made to the Phyllis Smith Memorial Fund c/o Fifth Third Bank, 915 Elliot Dr., Middletown, Ohio 45044.
